Edmonton Community Health Hub North Opening September 12

We are excited to announce that the Edmonton North Community Health Hub North (Hub), located at 13211 Fort Road NW, will be open Monday, September 12, 2022. The Hub will be home to the PCN’s current location at Northgate mall, along with Addictions and Mental Health and Continuing Care with Alberta Health Services. All of our current programs and services will be moving to the Hub.

Our current Northgate location will be closed as of Monday, September 5. If you have an in-person appointment that was at Northgate after September 12, it will be at the Hub. Our staff will be calling all patients who currently have an appointment after September 12, we will be contacting you directly to confirm the change in location.

The Hub has ample free parking and is located nine minutes east of our Northgate location.

This class is open for anyone to register for. No need for a referral from your family doctor.

 

Steps to register for a class:

  1. Find the class you would like to attend and call 780-473-7131.
  2. A few days before the class start date, you will receive an email with a link to the virtual group, along with an electronic copy of the class materials.
  3. You can also download the class materials from the class description, there is a link there.

If you are unable to attend, please be sure to give us a call at 780-473-7131 two business days prior so we can try to book someone else in your spot.

This class requires a referral from a primary care provider who is a member of the Edmonton North Primary Care Network. 

Steps to get a referral:

  1. Book an appointment with your primary care provider
  2. Ask for a referral to this class
  3. Our staff will contact you once we receive your referral to book you into this class
