Diabetes Management

If you are living with type 2 diabetes and are looking for support, education, monitoring, and building self-management skills for better blood sugar control, the PCN healthcare professionals can help. 

We can also support you if you have prediabetes or a family history of diabetes and are looking to prevent diabetes.

Diabetes Management Food Plate

This service is offered at:

  • Services at the Edmonton Community Health Hub North require a referral from your family doctor, who is a member of the PCN.
  • Services at Rundle Health Centre are available to you if you have a T5W, T5A or T5Y postal code. You do not require a referral. You can either call or walk in to book an appointment.

What to expect?

Once a referral is received from your family physician you will be booked for a Chronic Disease Initial appointment with one of our healthcare providers. This appointment is scheduled for one hour and can be done over the phone or in-person.

During this appointment the chronic disease clinician will review with you the factors that might be impacting your blood sugars and what you are currently doing to help manage them. They will also check for recent lab work relating to your concern and provide you with education on your health condition.

By the end of the appointment they will help you set some goals and develop a care plan. The health care provider will likely book a follow-up appointment to review your progress with goals and update your care plan. They will also check for follow-up blood work to see if the strategies used are making a difference. 

If you require more specialized care, they can also refer you to other health care providers at the PCN for further support.

How to Register for Classes?

Edmonton North PCN

This class is open for anyone to register for. No need for a referral from your family doctor.

 

Steps to register for a class:

  1. Find the class you would like to attend and call 780-473-7131.
  2. A few days before the class start date, you will receive an email with a link to the virtual group, along with an electronic copy of the class materials.
  3. You can also download the class materials from the class description, there is a link there.

This class requires a referral!

This class requires a referral from a primary care provider who is a member of the Edmonton North Primary Care Network. 

Steps to get a referral:

  1. Book an appointment with your primary care provider
  2. Ask for a referral to this class
  3. Our staff will contact you once we receive your referral to book you into this class
